I-693 Report of Immigration Medical Examination and Vaccination Record
The immigration medical exam. A USCIS-designated civil surgeon examines the applicant, checks vaccinations, and seals the report. It is not really filed by the applicant - the doctor seals it and the applicant submits the sealed envelope, or the doctor sends it electronically, with the green card application.

Qué cambió en 2025 y 2026
- Since December 2024 USCIS has treated a properly completed I-693 as having no expiry date, which removed a long-standing trap where medicals went stale while a case sat in a queue. Only a designated civil surgeon may sign it; a panel physician abroad is for consular cases.
